I'm fairly certain they have lost all of my documents. I sent my ID, birth certificate and marriage certificate through certified mail (which is the only way to file an application for a name change with this office's COVID rules) one month ago. They still have not opened this envelope. I have been calling, and they just keep sayingit's not in the systemeven though the USPS confirmed that the documents were delivered to the office. This is absolutely unacceptable and now I'm pretty sure I'm going to have to go through the process of recovering all of these important documents, all without having the name change completed. This has all meant that the name on the deed to my house will be incorrect, which I was assured by this office that they would be able to change my name in time because I was giving them over a month to open an envelope before my closing date. Awful.

I died u26b0 literally twice & have been denied 4x times already. The judge states and I quote, applicant does appear to have all signs of mental disability, bipolar disorder, manic depression disorder, severe anxiety & panic disorder, PTSD, COPD, Asthma, night tremors, applicant seems to speak in an unknown Possibly forgotten Latin language & able to move objects clear across the room shattering glass ware & bending medical tools without ever touching them physically. Several nights during sleep observation applicant was lifted up into the air several feet then placed back down. He suffers from severe insomnia, remedy is to continue sedatives (Ambien & Xanax) for night tremors. Patient does show some mental challenges but is not in any clear present danger. Conclusion does not qualify for SSI nor DSS. Patient is welcome to reapply. Applicant under observation shows that although he does have some mental limitations it is my professional opinion that the applicant will be just fine in public societyend quote.nJudge White W.P. I came here to fix an error on my social security and the lady told me that I needed to get a certificate of naturalization in order to change my name even though I already have a US passport because I am a derived citizen. They don't know how to do their jobs properly. Unacceptable!!

Very poor customer service. The representative talked to me like she was doing me an favour. Maybe she has forgotten that it's her job to explaine to people their rights and possibilities. Maybe she wasn't ever told that if you deal with humans, you must be polite. I just hope I will never visit this office again.

SS office made a spelling mistake in the last name on my social security card. I took back my SS card along with the birth certificate so they can correct it. The lady at the Window stated that even though they made a mistake with the name and even if I had a copy of my birth certificate they still want a letter from my Primary doctor stating that I am that person and this is my last name. Ridiculous! They cant be more stupid than that.I understand that every body make mistakes but why a letter from my primary.
